• By Mail: Complete the SSN update form you received from us and mail it to:
  Excellus BCBS, P.O. Box 22999, Rochester, NY 14692
• By Phone: Call the phone number on the back of your member id card
• By Email: Access our Secure Online Inquiry Form to submit electronically.

Note: If you have group insurance through NY State of Health, you can update or add a Social Security Number by sending us your completed form or you may contact NY State of Health directly. SHOP members may also contact their group administrator.
