Lower Premium:

Best Fit: Comprehensive adult/family coverage for cleanings, exams, fillings and more.

Healthy Smile Premier Dental
Best Fit: Same comprehensive adult/family coverage as the basic plan but with lower costs for Class II services (e.g., fillings).

Essential Plan
The Essential Plan is a free option for people with low or moderate incomes who don't qualify for Medicaid. The plan costs $0 and offers the same essential benefits as other health plans.
Medicaid & Child Health Plus
We offer several free and low cost health plan choices for individuals, families, and just for kids! Our state-sponsored programs include Medicaid Managed Care, Child Health Plus, and more.
Dental
Our dental plans for individuals and families include coverage for preventive and routine services like cleanings, fluoride treatments and sealants, exams, x-rays, root canals, and more.

If you qualify for financial assistance, you must apply through NY State of Health.

Eligibility Criteria for Young Adults Through Age 29
Young adult dependents are eligible for coverage until the end of the month in which they turn 30 years of age, if they:
- Are unmarried
- Are not insured by, or eligible for coverage under, an employer-sponsored health benefit plan as an employee or member, whether insured or self-insured, and
- Live, work, or reside in New York state or our service area
To be covered, they do NOT need to:
- Be a student, nor
- Live with or be financially dependent upon you
Note regarding disabled dependents:
You do not need to add this coverage for disabled dependents. If you are looking to cover a disabled dependent only, select ‘No’ for this question. You will need to complete and submit an Adult Disabled Dependent Form when you enroll in a plan. This form can be found on the Print Forms page. Your enrollment application cannot be submitted without this form.
